Form 1095-B Availability Notice

Form 1095-B will be available upon request beginning January 31, 2026. To request a copy, please email SHARP@NADAdventist.org.

Form 1095-B applies only to individuals under age 65 who are enrolled in healthcare benefits through Adventist Retirement.
